Job Summary

This is a PRN position that requires flexibility and a commitment to providing high-quality patient care in the comfort of the patient's own home.

Key Responsibilities
  • Provide technical skilled care to patients in their homes, following the plan of care and in coordination with the healthcare team.
  • Conduct thorough assessments and document patient progress, submitting daily reports to the agency.
  • Collaborate with patients and their families to educate them on proper care and maintenance of a healthy environment.
  • Monitor patient responses to treatment and report any changes in condition to the supervising nurse and/or physician.
Requirements
  • Current LPN licensure in the state of Kentucky.
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • CPR certification.
