Job Overview

Position Type: Per Diem

As a Per Diem Case Manager RN, you will receive compensation based on the number of visits, along with mileage reimbursement. Any hourly pay will be applicable for meetings, training sessions, or tasks assigned by the office. Separate on-call rates will apply if you are requested to cover calls.

This role has the potential to evolve into a full-time salaried position with benefits and opportunities for growth within the organization.

Key Responsibilities:
  • Deliver prescribed nursing care services to patients who are ill, recovering, or disabled in a home environment, working independently without direct supervision.
  • Utilize scientific knowledge from various disciplines to effectively apply the nursing process.
  • Exhibit proficiency in performing essential nursing skills, maintaining clinical competencies in accordance with established standards.
  • Administer medications and treatments in compliance with organizational policies and accepted care standards.
  • Engage in effective communication with patients, their families, and healthcare providers.
  • Conduct thorough assessments, develop and implement a comprehensive Plan of Care in collaboration with the patient, adhering to physician directives and guidelines.
  • Document patient status accurately and promptly, ensuring compliance with organizational policies regarding patient care, interventions, and education.
  • Demonstrate leadership and organizational skills by managing time and personnel effectively, supervising and mentoring support staff.
  • Foster a collaborative and supportive work environment by assisting colleagues and participating in team meetings.
  • Complete scheduled visits and partake in the on-call rotation as necessary.
Additional Duties:
  • Perform other responsibilities as assigned.
  • Maintain knowledge of Medicare/Medicaid and insurance standards to ensure appropriate reimbursement for services provided.
  • Adhere to all relevant organizational policies and procedures.
  • Engage in ongoing education and training opportunities.
  • Ensure confidentiality of patient information and business practices.
  • Take responsibility for reporting incidents and concerns in accordance with organizational policies.
