We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Avamere Suzanne Elise. As an LPN, you will play a vital role in providing high-quality care to our residents, ensuring their physical, emotional, and social needs are met.

• Plan, develop, and implement resident care plans in collaboration with the interdisciplinary team.
• Administer medications and review medication cards for accuracy and completeness.
• Conduct daily rounds to ensure staff is providing direct care in accordance with resident care plans and wishes.
• Develop and maintain rapport with nursing staff, residents, and family members.
• Participate in facility surveys and maintain accurate records.

• Current, unencumbered LPN license in the state of Oregon.
• Nursing degree from an accredited college or university.
• Knowledge of nursing and medical practices, laws, regulations, and guidelines.
