As a Registered Nurse (RN) Case Manager at Enhabit Home Health & Hospice, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. This is an exciting opportunity to join a national leader in home-based care and make a meaningful impact on the lives of our patients.
Key Responsibilities- Perform home visits and coordinate client care to ensure the best possible outcomes for our patients.
- Act as the point of contact for all disciplines involved in patient care, overseeing the frequency of visits and ensuring seamless coordination.
- Consult with physicians and office staff as needed to provide detailed updates on patient care.
- Accurately collect and document OASIS data, ensuring medication profiles are up-to-date and lab values are reported to physicians in a timely manner.
- Attend weekly case conferences and monthly case manager meetings to stay informed and share best practices.
- Current licensure as a Registered Nurse (RN) in the state of employment.
- A minimum of one year of clinical experience, with a preference for experience in home health care.
- Demonstrated knowledge and skill in current nursing practice.
- A valid state driver's license and automobile liability insurance.
