Key Responsibilities

  • Collaborate with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial intervention.
  • Develop and implement effective discharge plans to ensure seamless transitions for patients.
  • Provide education and support to patients and families to promote self-advocacy and empowerment.
  • Work closely with interdisciplinary teams to ensure comprehensive care and address complex patient needs.

Requirements

  • Master's in Social Work required.
  • LMSW required; LCSW/LCSW-C preferred.
  • 3-5 years of experience in inpatient social work or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
