About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Providence Health & Services. As a Clinical Pharmacist, you will play a critical role in providing high-quality, patient-centered care and integrated pharmacy operational services.

Key Responsibilities
  • Provide pharmaceutical care to patients, including medication therapy management and education.
  • Collaborate with the healthcare team to optimize patient medication therapy outcomes and quality of life.
  • Develop and implement pharmacy operational services, including medication use evaluations and formulary management.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
  • Stay up-to-date with the latest clinical and regulatory developments in pharmacy practice.
Requirements
  • Pharmacy Doctorate or Bachelor's Degree in Pharmacy.
  • California Pharmacist Registration upon hire.
  • Board of Pharmacy Preceptor license when eligible and where applicable.
  • Basic Life Support-Provider (BLS) certification where applicable.
Preferred Qualifications
  • Acute care hospital pharmacy experience, 1 year or equivalent experience.
  • Completion of ASHP-accredited PGY1 residency.
