Position Overview

We are looking for a skilled and compassionate Speech Language Therapist (SLT) to become a part of our versatile rehabilitation team at Fairview Health Services. The ideal candidate will possess formal education and hands-on experience in delivering feeding therapy services.   • Full-time equivalent (FTE) position, authorized to work up to 80 hours per pay period.

Fairview Health Services Rehabilitation provides a wide array of services catering to patients in multiple acute-care hospitals, post-acute settings, and outpatient therapy clinics. Key Responsibilities

The Speech Language Therapist collaborates with patients and families during the evaluation process:

Collects relevant information

Conducts assessments

Establishes treatment diagnoses

Creates a personalized care plan in partnership with the patient and family:

Analyzes data to identify needs and potential risks

Determines recovery prognosis

Sets individualized goals with the patient and/or caregiver

Customizes the care plan considering developmental and cultural factors

Implements and adjusts the care plan based on reassessment and patient feedback

Coordinates care and communicates effectively with the interdisciplinary team regarding all aspects of patient care

Develops appropriate discharge plans

Engages with patients and families to deliver skilled interventions:

Implements the care plan

Modifies treatment plans in response to changes in patient status

Evaluates patient and/or caregiver educational needs and provides necessary training

Documentation and Billing:

Utilizes accepted medical terminology

Adheres to professional practice standards

Completes documentation promptly according to departmental standards

Follows departmental billing protocols

Professional Expectations:

Prioritizes tasks and patient care for effective time management

Understands and complies with professional licensure and certification requirements

Leads team members and/or supervises students as assigned

Engages in personal and professional development to enhance clinical best practices

Qualifications

Required:

Certification from the American Speech-Language-Hearing Association (ASHA) and a Speech Language Pathology license in the state of Minnesota, unless in Clinical Fellowship Year.

Current Minnesota Basic Life Support certification.

For Speech Language Pathologists in Clinical Fellowship Year:

Active temporary license in Minnesota, ASHA membership, and actively pursuing the Certificate of Clinical Competence.

Preferred:

Completion of Clinical Fellowship Year

At least 1 year of experience
