Job OverviewThis role is part of the Healthy Start Grant, focusing on providing comprehensive support to perinatal patients. As a Nurse Navigator, you will work closely with multidisciplinary teams to deliver expert nursing care, addressing complex behavioral health and substance use disorder needs.
The ideal candidate will possess a strong educational background, preferably from a CCNE or NLN accredited nursing program. A Bachelor's degree in nursing is also highly desirable. Current licensure as a Registered Nurse in South Dakota or meeting multi-state licensure requirements is essential.
Responsibilities- Utilize the nursing process to provide high-quality patient care, adhering to the American Nurses Associations (ANA) scope and standards of practice.
- Collaborate with providers to ensure seamless patient care delivery.
- Develop and maintain professional relationships within the multidisciplinary team.
- Competitive hourly rate: $26.25-$39.25 per hour, dependent on experience.
