Job Summary

We are seeking a skilled Case Manager to join our team at Christus Health. As a Case Manager, you will play a critical role in ensuring patient progression through the continuum of care in an efficient and cost-effective manner.

Key Responsibilities
  • Collaborate with patients, families, physicians, and interdisciplinary teams to develop and implement individualized care plans.
  • Identify and initiate optimal patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, and length of stay management.
  • Provide comprehensive assessment, planning, implementation, and evaluation of patient needs.
  • Coordinate care and discharge planning to ensure seamless transitions.
  • Fulfill payer-based requests regarding individual members, where appropriate and pertinent.
Requirements
  • Graduate of an accredited school of nursing, BSN preferred, or Master's Degree in Social Work (MSW) required.
  • Licensed as a Registered Nurse to practice nursing in the state of New Mexico, or current licensure as an LMSW/LISW/LCSW in New Mexico.
  • Case Management certification highly desirable – ACM (Accredited Case Manager), CCM (Certified Case Manager, RN-BC (Board Certified in Case Management).
  • Ability to communicate effectively with diverse populations, function in a stressful and fast-paced environment, and possess interpersonal skills that promote teamwork.
  • Critical thinking/problem-solving skills, knowledge of and ability to utilize in-house and external resources, flexibility in prioritization, ability to multitask, and effective organizational skills.
  • Significant knowledge and understanding of availability of community and post-acute resources.
Experience

Two years' experience in a healthcare setting, with at least one year in an acute care setting, preferred.

Supervision

Responsible to: Director of Case Management.

Environment

General office environment and general hospital department/nursing units (100% of day).
