Responsibilities:
- Develop and implement individualized plans of care to meet the unique needs of each patient.
- Provide direct occupational therapy services, including evaluations, treatments, and education.
- Collaborate with other healthcare professionals to ensure comprehensive care.
- Maintain accurate records and documentation, including progress notes and discharge summaries.
- Participate in quality assurance and performance improvement initiatives.
Requirements:
- Degree in Occupational Therapy from an accredited program.
- Current licensure to practice as an Occupational Therapist in the state of employment.
- Minimum one year of experience in home health care or a related field.
- Strong communication and interpersonal skills.
