Key Responsibilities- Assess patients' physiologic health status and develop individualized plans of care
- Conduct patient interviews, explain policies and procedures, and review patient charts
- Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status
- Determine mobility, sensory deficits, prostheses use, and skin condition, and adjust plans of care as needed
- Communicate and document patients' physiologic health status and plans of care
- Collaborate with physicians and other healthcare team members to coordinate patient care
- Participate in patient and family education, and provide discharge instructions
- Perform related duties as required
- Graduate from an accredited School of Nursing
- Current license to practice as a Registered Professional Nurse in New York State
- BCLS and ACLS certification preferred
