Responsibilities:

  • Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
  • Collaborate with interdisciplinary teams to ensure seamless coordination of care and achieve optimal patient outcomes.
  • Administer medications and treatments as prescribed by physicians and participate in regular team meetings to discuss patient progress and address any concerns.
  • Counsel patients and their families on meeting their nursing and related needs, providing emotional support and guidance throughout their journey.
  • Maintain accurate and detailed clinical notes, communicating effectively with healthcare professionals and other stakeholders to ensure continuity of care.

Requirements:

  • Degree from an accredited nursing school and current licensure to practice in the state of operation.
  • Minimum 1 year of nursing experience, with 3+ years preferred.
  • Experience in hospice or similar settings a plus.
  • Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
