St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) is dedicated to providing comprehensive care for individuals aged 55 and older who are managing chronic illnesses or disabilities. Our mission is to ensure that these individuals can maintain their independence while receiving the necessary medical support in their homes and communities. The healthcare teams at St. Paul's PACE take a proactive approach in coordinating medical, social, and home care services, allowing participants to focus on their well-being.
This role is a full-time position within our Day Center, typically operating Monday through Friday from 8:00 am to 4:30 pm.
Key Responsibilities:
Under the guidance of the Day Center Supervisor, you will be responsible for assisting participants with their daily living activities, providing restorative and supportive care. Your duties will include:
- Facilitating and organizing activity programs for participants.
- Monitoring health conditions and ensuring a safe environment for all individuals.
Qualifications:
- Education: High school diploma or equivalent. Must possess a current Certified Nurse Assistant certification in California.
- Experience: At least one year of experience working with elderly or frail populations is required. Relevant training or experience in the assigned role is essential.
- Certifications: Current CPR (BLS) and First Aid certification from the American Heart Association are mandatory.
