Job Summary

Tenet Healthcare is seeking a highly skilled Social Worker to join our team as a Care Transition Specialist. This is a challenging and rewarding role that requires strong communication and problem-solving skills.

Key Responsibilities
  • Assess patients' psycho-social needs and develop effective transition plans to ensure seamless care coordination.
  • Collaborate with healthcare teams to identify and address potential barriers to care, including social determinants of health.
  • Provide education and support to patients and families to promote self-management and independence.
  • Lead and facilitate multi-disciplinary patient care conferences to ensure comprehensive care planning.
  • Develop and maintain relationships with community resources and providers to ensure access to necessary services.
  • Document patient information and progress in a timely and accurate manner.
Requirements
  • Master's degree in Social Work (MSW) from an accredited program.
  • Current licensure as a Social Worker in the state of practice.
  • 2+ years of experience in a healthcare setting, preferably in a case management or care coordination role.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
Preferred Qualifications
  • Accredited Case Manager (ACM) certification.
  • Experience working with vulnerable populations, such as the elderly or those with chronic illnesses.
Physical Demands

This role requires frequent sitting, standing, and walking, as well as the ability to lift up to 25 pounds. Manual dexterity and mobility are also required to perform the duties of the position.
