As a RN Case Manager, you will be responsible for coordinating patient care, developing and implementing individualized care plans, and collaborating with healthcare professionals to ensure seamless care transitions.
Responsibilities- Conduct comprehensive patient assessments to identify needs and develop individualized care plans
- Coordinate patient care services, including home health services, medical equipment, and transportation
- Collaborate with healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care
- Monitor patient progress, adjust care plans as needed, and communicate with patients and families to ensure satisfaction and understanding
- Participate in quality improvement initiatives to enhance patient outcomes and care delivery
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation
- One (1) year experience as a RN
- Required Certifications and Licensures: Licensed to practice as a registered nurse in the state of agency operation. Must possess and maintain valid CPR certification while employed in a clinical role. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
