We are seeking a highly skilled Physical Therapist to join our team in providing community-based outpatient services in the Batesburg-Leesville, SC area.
Job Summary:This is a full-time, part-time, or PRN Physical Therapist position to perform outpatient therapy within home-based settings. The successful candidate will provide high-quality, patient-centered care to older adults in their homes.
Responsibilities:- Provide community-based outpatient services to patients in their homes
- Develop and implement individualized treatment plans to meet the unique needs of each patient
- Conduct thorough assessments and evaluations to identify patient needs and goals
- Collaborate with multidisciplinary teams to ensure comprehensive care
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
- Graduation as a Doctor of Physical Therapy (or equivalent standing)
- Eligibility for South Carolina state licensure as a Physical Therapist
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
