Admissions Screener

The McGuire Group Inc is seeking a skilled Admissions Screener to join our team supporting our facilities in Erie County.

This exciting opportunity involves coordinating all admissions and transfers to the facility, working closely with nursing staff and social services to meet prospective residents' needs.

• Screen and recommend patients for long-term or subacute care based on age-specific developmental factors.
• Visit designated hospital assignments daily to assess patient needs and facilitate smooth transitions.
• Coordinate room changes and inter-facility transfers upon request and approval from resident physicians.
• Interact with residents, family members, coworkers, and clinical/ancillary staff in a non-judgmental, supportive manner.

• Bachelor's degree in Social Work, Nursing, Therapy, or a related health field from an accredited institution.
• Experience in Admissions Screening and long-term care is preferred.
• Must be flexible, professional, and reliable.
• Strong customer service skills.
• Possess a valid New York State Driver's License and have transportation available for job-related travel.

Competitive salary: $26-$36 per hour, depending on license and experience.
