Key Responsibilities
  • Conduct comprehensive bio-psychosocial assessments to identify client needs and develop personalized treatment plans.
  • Lead group and individual therapy sessions, teaching coping mechanisms and facilitating safe discharge plans.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and timely documentation.
  • Prepare and submit written reports and case summaries, adhering to program standards and professional ethics.
  • Utilize electronic medical records (EMR) to document client progress and maintain accurate records.
  • Participate in treatment teams and contribute to the development of innovative care strategies.
Requirements
  • Must be 21 years or older.
  • Must possess a valid driver's license.
  • Ohio Medicaid billing number and NPI number required.
  • Minimum of an associate's degree and Ohio licensure to practice.
  • Current licensure as an LSW, LISW, LPC, LPCC, LCDC II, LCDC III, or LICDC.
