About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Hospice team as a RN Case Manager. As a key member of our interdisciplinary team, you will be responsible for delivering high-quality, patient-centered care to individuals and their families in need of end-of-life support.

Responsibilities
  • Conduct initial nursing evaluations to determine eligibility for hospice services
  • Develop and implement individualized plans of care, collaborating with patients, families, and healthcare providers
  • Provide ongoing assessment and evaluation of patient needs, adjusting care plans as necessary
  • Document patient progress, including problems, goals, interventions, and outcomes
  • Collaborate with the patient's attending physician and other members of the IDG to ensure comprehensive care
Requirements
  • Current RN licensure in the state of practice
  • One year of clinical experience, preferably in hospice or palliative care
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
