The Phoenix at Lake Lanier is seeking a Wellness Nurse Specialist to join their team in the picturesque surroundings of Lake Lanier. As a key member of the healthcare team, the successful candidate will be responsible for monitoring the health and wellbeing of residents, providing leadership support and assistance with resident care and services.

Responsibilities include:
  • Monitoring health and wellbeing of residents through regular assessments and personalized service plans (PSP)
  • Ensuring proper follow-through and documentation for residents with changes in clinical care needs
  • Assisting the Wellness Director with completing clinical sections of monthly wellness visits and PSP to accurately reflect resident needs
  • Informing the Wellness Director and/or Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-out
  • Assisting with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates
  • Providing direct resident care as needed

Requirements:
  • A minimum of one year of experience working with elders or disabled individuals, preferred
  • Must be a Licensed Practical Nurse or Registered Nurse
  • Demonstrated leadership capabilities
  • Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred
  • Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times

Estimated Salary: $65,000 - $85,000 per annum
