We are seeking a highly skilled Clinical Pharmacist to provide expert clinical pharmacy services to patients in critical care and complex care settings at Mass General Brigham.
The successful candidate will be responsible for optimizing drug therapy, medication reconciliation, and patient education to improve patient outcomes.
Key Responsibilities- Provide direct patient care and clinical pharmacy services to patients in critical care and complex care settings
- Develop and implement pharmaceutical care plans to improve patient outcomes
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
- Participate in quality improvement initiatives to enhance patient safety and care
- Eligibility for licensure to practice pharmacy in the Commonwealth of Massachusetts
- Postgraduate residency/fellowship training preferred
- Board Certification by examination, or equivalent certification, in the relevant specialty preferred
- Strong knowledge and skills in clinical pharmacy practice, including medication therapy management and patient education
