We are seeking a highly skilled and compassionate Mental Health Therapist to join our team at Community Health Network. As a Behavioral Health Specialist, you will play a critical role in providing comprehensive mental health services to individuals and families in our community.
Key Responsibilities- Facilitate, monitor, and provide comprehensive mental health/chemical dependency case management and therapy services to addicted and/or severely mentally ill adults or seriously emotionally disturbed children and their families.
- Collaborate with multi-disciplinary treatment teams in planning, facilitating, and coordinating treatment, especially in the areas of assessment, referral, and discharge planning.
- Identify needs and provide links through case management to other treatment programs, community resources, services, and entitlements.
- Develop and implement effective and efficient treatment discharge plans.
- Provide services in homes, schools, or other community-based sites, and provide transportation of clients in personal vehicles.
- Actively involved in service line quality improvement efforts.
- Provide psychological first aid in response to a disaster or critical incident, if trained to do so.
- License eligible for the LCSW, LMHC, LMFT, or LCAC in the State of Indiana required.
- Master's degree from an accredited college or university in a mental health related field required.
- 2+ years of experience providing mental health treatment required.
- Possess current Indiana Driver's license with proof of current auto insurance.
- Recognizes and commits to critical importance of customer satisfaction.
