Job Summary

The Outpatient Registered Dietitian is a key member of our multidisciplinary healthcare team at Cheshire Medical Center. This role is responsible for providing education and counseling to patients with diabetes and other chronic diseases.

Responsibilities
  • Develop and implement individualized medical nutrition therapy and education plans for patients with diabetes and other chronic diseases.
  • Assess and provide education to patients on relevant medical history, cultural influences, health beliefs and attitudes, diabetes knowledge, self-management skills and behaviors, readiness to learn, cognitive ability, physical limitations, family support and financial status.
  • Counsel and educate individual patients on diabetes care and management.
  • Prepare and update Diabetes program policies and procedures.
  • Participate as a member of the interdisciplinary healthcare team to facilitate the continuum of care and education of patients with diabetes and other chronic diseases.
Requirements
  • Bachelor degree in Nutrition.
  • A minimum of 3-5 years experience of individual nutrition counseling.
  • Experienced with individual counseling and presenting community education programs.
  • Academic preparation and or experience in program management.
  • Commission on Dietetics Registration as RD and NH licensure required upon hire.
  • Certified Diabetes Educator by the National Board for Diabetes Educators preferred.
