KEY RESPONSIBILITIES:
The Geriatric Care Physician (GCP) is a licensed and Board Certified/Board Eligible expert in internal or family medicine, playing a vital role within our clinical operations team. This position involves delivering direct patient care and conducting assessments primarily in outpatient settings, with occasional responsibilities in acute care, nursing homes, skilled nursing facilities, and home environments, depending on the assignment.
Responsibilities encompass but are not limited to: comprehensive geriatric evaluations, thorough medical histories, physical examinations, accurate diagnoses and treatments, care plan development, health education, specialty referrals, case management referrals, follow-ups, and meticulous documentation in accordance with ChenMed's standards for quality, service, productivity, and teamwork. The GCP will also engage in clinical rounds and conferences, ensuring detailed documentation through written progress notes and summaries.
The GCP is expected to demonstrate the capability to operate both independently and collaboratively with other healthcare professionals. They will consult with relevant managers and medical directors to ensure adherence to guidelines and participate in risk and quality management programs, clinical meetings, and other necessary gatherings.
KEY ROLES AND PERFORMANCE MEASURES
Clinical Practice (65% of responsibilities)
- Function independently as a primary care practitioner within a patient care team.
- Assess acute and non-acute clinical issues independently.
- Perform and document physical assessments and patient histories, analyze patient condition trends, and develop, document, and implement patient management plans based on collected data.
- Provide appropriate counseling and education to patients and their families.
Leadership (5% of responsibilities)
- Utilize advanced communication skills to resolve complex situations and enhance processes and services for patients.
- Collaborate with multidisciplinary team members to evaluate current healthcare delivery systems and implement new practice patterns as needed.
Continuity/Transition Management (20% of responsibilities)
- Initiate discharge arrangements and complete necessary paperwork.
- Collaborate with physicians, nurses, social workers, and caregivers to ensure smooth transitions to lower levels of care when medically appropriate.
Advocacy & Education (5% of responsibilities)
- Introduce oneself to patients and families, clarifying the role of the primary care provider.
- Facilitate family conferences to review treatment goals and optimize resource utilization.
Utilization/Financial Management (5% of responsibilities)
- Manage resource utilization and reimbursement for services effectively.
QUALIFICATIONS:
- Unrestricted MD or DO License in Florida
- Board Certification in Internal Medicine or Family Practice
- Fellowship in Geriatrics is advantageous
- A minimum of one year of experience in Primary Care is preferred
- Experience in Managed Care is preferred
