Family Medicine Physician

Optum NY, a leading healthcare organization, is seeking a highly skilled Family Medicine Physician to join our team in Port Jefferson, NY. As a Family Medicine Physician, you will be an integral part of our vision to make healthcare better for everyone.
Key Responsibilities:- Provide high-quality, patient-centered care to adults, focusing on value-based care and supported by a robust team of specialists.
- Direct and manage a complete continuum of care within the care team, treating chronic diseases and disorders.
- Review patient files, including allergies, problems, medications, and immunization status, and order or execute various tests and diagnostic images to provide information on patient conditions.
- Analyze reports and findings of tests and examinations, and diagnose patient conditions.
- A physician-led, patient-centered team-based environment.
- Full-time, work-life balance, and flexible scheduling.
- Potential for limited inpatient/hospital rounding as part of a full-time, 40-hour per week schedule.
- Autonomy to practice at the peak of your license.
- Supported to grow your practice and patient panel.
- Must be comfortable working with Advanced Practice Clinicians.
- Unrestricted licensure in the state of New York or has New York application in process.
- Residency trained and Board Certification or Board Eligibility in Family Medicine.
- Active and unrestricted DEA License or ability to obtain prior to start.
