Key Responsibilities- Conduct in-home health assessments on enrolled Medicare Advantage members
- Perform annual health assessments, post-discharge visits, and visit complex members more than once per year
- Check vitals, conduct physical exams, and perform point-of-care testing as needed
- Identify diagnoses and formulate a list of current and past medical conditions
- Communicate findings to inform member's PCP of potential gaps in care
- Intervene in urgent and emergent situations
- NP license and national certification (ANCC or AANP) or ability to obtain
- Active, unencumbered licensure with no open/unresolved disciplinary actions
- Family, Geriatrics, Adult, or Adult-Gerontology Primary or Acute Care certification
- Access to reliable transportation and ability to travel up to 85% within licensed states
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Home Health care or home visit experience
