Key Responsibilities- Data Collection: Gather and document resident data, ensuring accurate and timely information.
- Implementation: Administer and delegate resident care services, ensuring quality and adherence to policies and procedures.
- Medication Administration: Safely and accurately administer medications, monitoring resident responses and reporting any concerns.
- Treatments and Clinical Skills: Perform various treatments and clinical skills, such as skin care, catheterizations, and tube feedings, as needed.
- Documentation: Maintain accurate and complete documentation, adhering to current standards and policies.
- Discharge Planning: Collaborate with the Registered Nurse and discharge planners to ensure smooth resident transitions.
- Emergency and Disaster Measures: Respond to emergencies and disasters, following established policies and procedures.
- Education: Graduate of a licensed practical nurse accredited program.
- Licensure: Current active Licensed Practical Nurse (LPN) nursing license in the State of Minnesota in good standing.
- Certifications: CPR certified.
- Communication: Strong written and verbal communication skills, with the ability to read, write, and communicate in English.
- Experience: Clinical experience with geriatrics, long-term care, and/or rehabilitation preferred.
- Regularly required to stand, bend, and walk.
- May also be required to sit, climb, stoop, kneel, crouch, push, and pull.
- Regularly assigned to lift or move objects up to 10 pounds.
- Frequently required to support up to 10-50 pounds.
- Emergency situations could require carrying/supporting 25 pounds or greater.
- Requires employee use hands for repetitive actions such as simple grasping, firm grasping, and fine manipulating.
- Requires functional hearing ability, read medical records, make observational assessments, and document in writing and computer records.
