Job Description

Prestige Home Care Agency is seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care services in the comfort of clients' homes throughout the Philadelphia area and surrounding counties.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work independently and as part of a team. Responsibilities will include:

  • Providing skilled nursing care and treatment to patients in their homes
  • Conducting routine assessments and documenting medical findings
  • Observing patient health and vital signs, and communicating with patient family and healthcare team members
  • Developing and implementing individualized care plans to meet patients' unique needs

Requirements:

  • Current LPN licensure in the state of Pennsylvania
  • Ability to drive throughout the Philadelphia area for home visits
  • Valid driver's license required
  • Prior LPN and skilled service experience preferred, but not required
