Job Title: Medical Care Team Member

First Connect is seeking a dedicated and compassionate Local Licensed Practical Nurse to join our medical care team. As an LPN, you will provide essential healthcare services to inmates in the Curran-Fromhold Correctional Facility (CFCF), promoting their physical and mental well-being.

About the Opportunity:
This is a full-time position working various shifts, including day and evening hours. You will be part of a multidisciplinary team responsible for delivering high-quality patient care, conducting health assessments, developing care plans, and implementing interventions to improve patient outcomes. Our ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for delivering exceptional care.

Key Responsibilities:
- Conduct thorough health assessments, identify patient needs, and develop individualized care plans
- Implement evidence-based interventions to promote patient well-being and prevent complications
- Collaborate with interdisciplinary teams to coordinate care, address concerns, and optimize outcomes
- Maintain accurate and detailed records of patient interactions, treatments, and progress

Requirements:
- Current LPN licensure in the state of employment
- Minimum 1 year of relevant experience in a healthcare setting, preferably in corrections or a similar environment
- Strong clinical knowledge, critical thinking skills, and ability to work independently
- Excellent communication and interpersonal skills, with the capacity to build rapport with patients, colleagues, and management
