Responsibilities
  • Evaluate patients' physical and functional status, as well as their cognitive, psychological, and perceptual abilities.
  • Document all evaluations, treatment, and coordination of care contacts via appropriate facility policies.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Provide education and counseling to patients and their families regarding treatment plans, goals, and potential safety risks.
  • Manage own caseload to maximize effectiveness of overall care plan.
Requirements
  • Bachelor's or higher degree in Physical Therapy from an institution accredited by Commission on Accreditation in Physical Therapy Education.
  • Current State Physical Therapist License.
  • Current CPR certification.
