Field-based Nurse Practitioner

CenterWell Senior Primary Care is seeking a highly skilled and compassionate Field-based Nurse Practitioner to join our team. As a key member of our care team, you will provide high-quality, patient-centered care to seniors in their homes, promoting health, wellness, and independence.
Responsibilities- Provide comprehensive primary care services to patients in their homes, including health assessments, diagnosis, treatment, and management of acute and chronic conditions.
- Develop and implement individualized care plans to meet the unique needs of each patient, taking into account their medical history, lifestyle, and social determinants of health.
- Collaborate with other healthcare professionals, including physicians, social workers, and home health aides, to ensure seamless care coordination and optimal patient outcomes.
- Conduct regular health screenings, provide health education, and promote preventive care to patients and their families.
- Prescribe medications, order laboratory tests, and refer patients to specialists as needed.
- Document patient interactions, progress, and outcomes in a timely and accurate manner, using electronic medical records (EMRs).
- Master's degree in nursing or related field.
- Current, unrestricted state license to practice as a Nurse Practitioner.
- Board certification in a relevant specialty (e.g., family medicine, internal medicine, gerontology).
- Minimum 2 years of experience as a Nurse Practitioner, preferably in a primary care or home health setting.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with a focus on patient-centered care and population health management.
- Proficiency in electronic medical records (EMRs) and other healthcare technologies.
- Experience working in a home health or primary care setting.
- Knowledge of gerontology and the unique needs of older adults.
- Experience with care coordination and population health management.
