Key Responsibilities- Medication Reconciliation and Optimization: Conduct thorough medication reconciliations for patients transitioning from acute care settings, identifying potential medication-related problems and making recommendations to optimize medication regimens.
- Collaboration with Healthcare Teams: Work closely with our healthcare teams, including physicians, nurses, and other pharmacists, to ensure seamless care transitions and optimal medication use.
- Medication Therapy Management: Develop and implement medication therapy plans, including medication selection, dosing, and monitoring, to ensure safe and effective use of medications.
- Patient Education and Counseling: Provide patients and their caregivers with education and counseling on medication use, including proper administration, potential side effects, and storage.
- Performance Improvement and Quality Assurance: Participate in performance improvement activities, including data collection, analysis, and implementation of quality improvement initiatives to enhance patient care and outcomes.
- Education and Licensure: Bachelor of Science in Pharmacy or Pharm.D. degree, with current and unrestricted Pharmacist license in the state of IN.
- Experience: Completion of a PGY1 pharmacy residency, plus at least 3 years' experience working in ambulatory healthcare delivery organizations, or completion of both a PGY1 pharmacy residency and a PGY2 ambulatory care pharmacy residency.
- Certifications: Board Certification (BCACP, BCPS, or BCGP) required.
- Skills and Abilities: Ability to calculate appropriate medication dosages based on pharmacokinetic parameters, identify significant drug interactions, and work effectively in a team environment.
