Key Responsibilities- Conduct comprehensive health assessments on patients, including physical exams, medical histories, and diagnostic tests.
- Develop treatment plans and prescribe medications to patients based on their individual needs.
- Provide patient education and counseling on disease prevention, healthy lifestyle choices, and other health-related topics.
- Collaborate with other healthcare professionals to ensure the highest level of patient care.
- Maintain accurate and up-to-date medical records and documentation.
- Participate in quality improvement initiatives and other clinical projects as needed.
- Must be a licensed Nurse Practitioner/Physician Assistant with previous experience in a Family Med, Community Health, or Outpatient setting.
- Any experience with Community Health is a plus.
- Strong clinical skills and knowledge of primary care practices and procedures.
- Excellent communication and interpersonal skills.
- Ability to work well in a team environment and collaborate effectively with other healthcare professionals.
- Strong attention to detail and ability to maintain accurate and up-to-date medical records.
- Ability to work independently and manage multiple tasks and responsibilities.
