Modern HR is seeking a skilled and compassionate Registered Nurse (RN) to join our team at GEM Transitional Care Center. As a NOC RN, you will be responsible for providing high-quality patient care, assessing and evaluating patient status, and developing individualized care plans.
Key Responsibilities- Provide Direct Patient Care: Assess and evaluate patient status, develop and implement individualized care plans, and provide direct patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation.
- Collaborate with Interdisciplinary Team: Work cooperatively with other members of the facility's interdisciplinary team to ensure comprehensive patient care.
- Supervise LVNs and CNAs: Supervise and direct the care provided by Licensed Vocational Nurses and Certified Nursing Assistants.
- Administer Medications and Treatments: Administer IV medications and treatments as prescribed by physicians.
- Document Patient Care: Document assessments and care in compliance with standards of care and company policy.
- Current RN License: Current RN license in the state of CA; New Graduates encouraged to apply.
- CPR Certification: Maintain current CPR certification.
- Leadership Experience: Previous experience supervising LVNs and CNAs.
- Excellent Communication Skills: Demonstrates excellent observation, problem-solving, verbal, and written communication skills.
- Autonomy: Ability to work with minimal supervision.
