Clinical MDS Specialist

Position Overview

Westford House Health Center, an esteemed affiliate of Best Care, is seeking a dedicated MDS Nurse. This role involves the critical responsibility of executing and submitting precise and timely MDS Assessments, CCAs, and Care Plans in alignment with CMS RAI Manual Regulations, while adhering to all relevant laws and Best Care standards.

Qualifications
  • Completion of an accredited LPN or RN program.
  • Active licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing within the applicable State.
  • A minimum of two years of nursing experience, with a preference for experience in geriatric nursing.
  • Certification in CRN C (clinical compliance).
  • Current CPR certification upon hire or obtained during orientation, with ongoing maintenance required.
  • Familiarity with Point Click Care (PCC) and PDPM.
Job Responsibilities
  • Conduct and submit MDS Assessments, CCAs, and Care Plans accurately and punctually as per CMS RAI Manual Regulations.
  • Notify the Director of Nursing (DON) of any changes in a patient's condition identified through the MDS Assessment.
  • Educate direct care associates on updates or modifications to the CMS RAI Manual or Skilled Nursing Facility Regulations that affect documentation.
  • Assist in the evaluation of the Interdisciplinary Comprehensive Care Plan.
  • Review Final Validation Reports to ensure all assessments are accepted into the CMS QIES system prior to billing, and inform the Business Office of any assessments not ready for billing.
  • Analyze CMS Reports to identify late submissions and develop strategies to prevent recurrence.
  • Participate in Daily PPS Meetings, Monthly Triple Checks, and other meetings as needed in the absence of the RN MDS Coordinator.
  • Perform nursing duties as required.
  • Exhibit exceptional customer service and maintain a positive demeanor towards patients.
  • Assist in patient evacuation when necessary.
  • Demonstrate reliable and consistent attendance.
  • Utilize reasoning skills and sound judgment in all situations.
