We are seeking a compassionate and skilled Palliative Care Nurse Practitioner to join our team at Agape Care Group. Responsibilities
  • Provide comprehensive patient assessments, develop and implement individualized care plans, and manage symptoms to improve patients' quality of life.
  • Collaborate with healthcare teams to ensure seamless care transitions and effective communication.
  • Educate patients and their families on disease management, treatment options, and self-care techniques.
Requirements

To be successful in this role, you must have:

  • A Graduate degree from an accredited nursing program (BSN or MSN).
  • At least 2 years of experience in chronic disease management, long-term care, or hospice settings.
  • Reliable transportation, ability to sit, stand, bend, move intermittently, and lift at least 25lbs.
Compensation and Benefits

We offer a competitive salary range of $120,000 - $150,000 per year, depending on experience, plus a comprehensive benefits package that includes tuition reimbursement, paid time off, employee referral program bonus eligibility, matching 401K, annual merit increases, and more.
