Developmental Disabilities Supports Coordinator

We are seeking a highly motivated and compassionate Supports Coordinator to join our team at Service Access & Management, Inc. As a Supports Coordinator, you will play a vital role in providing service coordination to individuals with Autism and Developmental Disabilities, supporting their vision of a good life.
The primary purpose of this position is to assist individuals in navigating the Office of Developmental Program (ODP) service system, accessing supports and services, and providing information to individuals and families. You will collaborate with individuals and team members to develop comprehensive plans that outline the strengths, support needs, and goals for individuals served.
Key Responsibilities:
- Assessment: Conduct initial and ongoing evaluations of the strengths and needs of individuals served.
- Planning: Collaborate with individuals and team members to develop comprehensive plans that outline the strengths, support needs, and goals for individuals served.
- Referral Coordination: Locate services and supports and connect individuals with identified supports to meet their individualized needs and help them achieve their goals.
- Monitoring & Support: Regularly monitor individual progress and provide ongoing support, guidance, and encouragement to ensure that support plans are being adhered to.
- Documentation: Maintain accurate and up-to-date records of individual progress, support plans, and any significant changes or developments, while also adhering to legal and ethical standards of confidentiality.
Requirements:
- A bachelor's degree, which includes 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science; or
- A combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science and two years of professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work, or health and rehabilitative services.
Working Conditions:
Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, and travel throughout the assigned region may occur and may be in adverse weather conditions, must be able to lift to 15 pounds.
