Position Overview:
The Licensed Practical Nurse (LPN) plays a vital role within the nursing services framework in a skilled nursing environment. As part of an interprofessional team, the LPN contributes to the resident and family education program through health teaching, counseling, and the provision of supportive care under the guidance of a registered nurse or licensed clinician.

  • Deliver holistic care to residents by implementing the nursing process systematically and in accordance with established protocols.
  • Execute clinical skills and services as per organizational policies and within the LPN scope of practice.
  • Complete necessary clinical competencies, certifications, and peer evaluations.
  • Collaborate with residents, families, and multidisciplinary teams to ensure continuous and coordinated care aligned with discharge objectives.
  • Respond effectively in emergency situations.
  • Assess and document resident progress, reporting significant observations to the healthcare team.
  • Communicate proactively regarding changes in patient conditions and care plans.
  • Maintain accurate and legible documentation of resident care and safety checks.
  • Monitor residents' physical and mental health, alerting the team to any abnormalities.
  • Educate residents on medications, procedures, and treatment protocols.
  • Administer medications through various routes as permitted by practice standards.
  • Provide enteral feedings as necessary.
  • Perform bedside care, including wound management and injections.
  • Measure and document vital signs as required.
  • Supervise certified nursing assistants and support staff.
  • Receive and document verbal orders from healthcare providers appropriately.
  • Implement approved medication changes as per pharmacy recommendations.
  • Initiate and discontinue peripheral IV access as needed.
  • Facilitate prescription orders for discharging residents.
  • Ensure a safe and welcoming environment for residents and visitors.
  • Adhere to infection control and safety protocols.
  • Engage in ongoing professional development through training and educational programs.
  • Maintain current CPR and IV certifications.
  • Assist in the discharge process to home or other healthcare facilities under supervision.
  • Actively participate in quality assurance and performance improvement initiatives.

Qualifications:
This position requires a full-time commitment with day shifts, rotating weekends, and holidays, typically encompassing 12-hour shifts.

  • Must possess an active New Jersey LPN license and BLS certification.
  • A minimum of two years of experience in post-acute care settings is preferred.
