Triage RN Job Summary

We are seeking a skilled and compassionate Triage RN to join our team at Mosaic Community Health. This role is a vital part of our ambulatory care team, providing high-quality patient care and support to patients across Central Oregon.

Key Responsibilities

  • Patient Care and Support
    • Provide high-quality patient care through telephone triage and assessment
    • Develop and implement personalized plans of care for each patient
  • Clinical Expertise and Decision-Making
    • Use sound clinical judgment to make decisions regarding patient care
    • Consult with other healthcare professionals as needed to ensure optimal patient outcomes
  • Communication and Documentation
    • Communicate effectively with patients, families, and healthcare teams
    • Accurately document patient interactions and care plans in the EHR

Requirements

  • Education
    • Bachelor's degree in Nursing (required)
  • Licenses and Certifications
    • CPR certification
    • Active RN license in Oregon
  • Experience
    • At least 2 years of experience in an ambulatory care setting (required)
    • Experience in telephone triage and patient education (preferred)

Estimated Salary: $80,000 - $100,000 per year
