**About the Role**

We are seeking a highly skilled Clinical Specialist to join our team at Haven Hospice. This role is responsible for providing clinical expertise in nursing management, documentation, and care planning.

Key Responsibilities:

  • Process start of care workflow once start of care visit has been completed.
  • Participate in IDG and review visits plotted on SOC or Re-cert visit and adjust as necessary.
  • Maintain orders in HCHB and manage admissions, deaths, discharges.
  • Assist with data collection for compliance, quality assurance, implementation of nursing policies and procedures, and staff development.
  • Uphold effective communication with referral sources to ensure smooth transition of admissions.

Requirements:

  • Associate degree in Nursing with current license.
  • Three years practice as a Registered Nurse.
  • Previous Hospice experience preferred.
  • Knowledge of the Hospice Philosophy of Care.
  • Ability to use Microsoft Word, Excel, Outlook, and other software programs.
