VITAS Healthcare is seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Care Specialist. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families facing terminal illness and bereavement.
Job Summary:The Registered Nurse, RN, will be responsible for coordinating care, assessing patient needs, and implementing the interdisciplinary plan of care. You will work closely with patients, families, caregivers, and healthcare professionals to ensure seamless care delivery and optimal patient outcomes.
Responsibilities:- Case Management and Supervision: Provide supervision and guidance to Hospice Aides, Licensed Practical/Vocational Nurses, and Volunteers to ensure high-quality patient care.
- Patient Assessment and Care Planning: Conduct comprehensive assessments, develop individualized plans of care, and implement interventions to address patient needs.
- Communication and Collaboration: Maintain effective communication with patients, families, caregivers, and healthcare professionals to ensure coordinated care delivery.
- Education and Training: Provide education and training to patients, families, and caregivers on end-of-life care, pain management, and other relevant topics.
- Quality Improvement: Participate in quality improvement initiatives and activities to enhance patient care and outcomes.
- Current licensure to practice nursing in the state where the VITAS program is located.
- A minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
- Equivalent experience or licensure may be considered.
The Registered Nurse, RN, will work in a fast-paced, dynamic environment, providing care to patients in various settings, including inpatient and continuous care units. The role requires excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment.
