Pediatric Care Coordinator

The RN Care Manager is a key member of the inpatient care team, responsible for coordinating patient care and ensuring seamless transitions between hospital and home. This role requires strong clinical knowledge, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Responsibilities- Coordinate patient care plans with the interdisciplinary team, including physicians, nurses, and other healthcare professionals.
- Conduct thorough assessments of patients' medical and social needs, identifying areas for improvement and developing strategies for optimal care.
- Collaborate with patients and families to ensure informed decision-making and active participation in care planning.
- Develop and implement effective discharge plans, ensuring timely and safe transitions to home or other care settings.
- Monitor and evaluate patient outcomes, making adjustments to care plans as needed to achieve optimal results.
- Minimum two years of nursing experience in a pediatric acute care or ambulatory setting.
- Strong clinical knowledge and skills in pediatric nursing, including assessment, diagnosis, and treatment of common pediatric conditions.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently and as part of a team, with strong problem-solving and critical thinking skills.
- Proficiency in Microsoft Office and other relevant software applications.
- Three years of nursing experience in an emergency department and/or case management, care coordination, or transition of care.
- Previous pediatric experience and/or management, and/or charge nurse role acceptable.
- Certification in Utilization Review and/or Case Management.
The RN Care Manager will work in a fast-paced inpatient setting, with frequent interactions with patients, families, and healthcare teams. The ideal candidate will be able to work effectively in a dynamic environment, with strong organizational and time management skills.
