A Safe Haven Foundation is seeking a compassionate and dedicated Licensed Clinical Social Worker to join our Adolescent – Intensive Outpatient Program. As a key member of our multidisciplinary team, you will provide therapeutic support and counseling services to adolescents struggling with behavioral health challenges.

Responsibilities:
  • Clinical Assessment and Treatment: Conduct comprehensive psychosocial assessments to evaluate clients' needs, strengths, and goals. Develop individualized treatment plans in collaboration with clients, utilizing evidence-based therapeutic modalities and interventions to address behavioral health issues.
  • Individual and Group Therapy: Conduct individual therapy sessions with adolescents to address mental health issues, such as mood disorders, anxiety, eating disorders, and substance abuse. Lead group therapy sessions focused on specific issues relevant to the participants.
  • Case Management: Coordinate care for adolescents, including developing treatment plans, monitoring progress, and adjusting plans as necessary. This also involves working with families, schools, and other involved parties to ensure comprehensive care.
  • Crisis Intervention: Respond to immediate crises, such as severe emotional distress or self-harm. Provide support, stabilization, and necessary referrals to higher levels of care when needed.
  • Family Counseling: Engage with families in therapy sessions to address family dynamics, communication, and other issues that may be impacting the adolescent's mental health.
  • Collaboration with Multidisciplinary Team: Work alongside other mental health professionals, including psychiatrists, psychologists, nurses, and counselors, to provide coordinated care. Participate in regular team meetings to discuss cases and treatment strategies.
  • Documentation and Compliance: Maintain accurate and timely records of all therapy sessions, assessments, treatment plans, and communications with other professionals. Ensure that all documentation complies with state and federal regulations as well as organizational policies.
  • Education and Advocacy: Educate adolescents and their families about mental health issues, treatment options, and coping strategies. Advocate for the needs and rights of the adolescents within the program and in broader contexts like schools or community services.
  • Discharge Planning: Assist with the development of a discharge plan when adolescents are ready to transition out of the IOP. This includes ensuring continuity of care, making referrals to appropriate services, and preparing the adolescent and their family for the transition.
  • Professional Development: Stay abreast of current research, best practices, and trends in the field of behavioral health and social work. Engage in ongoing professional development activities, including training, supervision, and peer consultation, to enhance clinical skills and knowledge.
  • Clinical Oversight of QMHP: Regularly review and discuss cases the QMHP is managing to ensure appropriate treatment planning. Support in making clinical decisions, especially in complex cases, aligning with best practice and ethical standards. Regularly evaluate the performance and help set professional development goals, which may include preparation for the licensing exam in the future.
Qualifications:
  • Master's degree in social work, counseling, or a related field from an accredited program.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LCPC) in the state of Illinois.
  • A minimum of 5 years of clinical experience providing therapy and counseling services to individuals with behavioral health disorders, preferably in a community mental health or outpatient setting.
  • Experience in working with adolescent clients.
  • Strong clinical assessment and diagnostic skills, with the ability to formulate accurate clinical impressions and treatment plans based on client needs.
  • Proficiency in delivering evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), motivational interviewing (MI), and trauma-informed care.
  • Excellent communication and interpersonal skills, with the ability to establish rapport with clients, build therapeutic alliances, and collaborate effectively with colleagues and stakeholders.
  • Commitment to cultural competence, diversity, equity, and inclusion, with the ability to work effectively with clients from diverse backgrounds and identities.
  • Ability to work independently and as part of a team in a fast-paced, dynamic environment, demonstrating flexibility, adaptability, and resilience.
  • Knowledge of ethical and legal standards governing the practice of social work and counseling, including confidentiality, informed consent, and mandated reporting requirements.

Work Schedule: Monday thru Friday, 8 am - 4 pm
