As an Independent Optometrist leasing space inside our Walmart stores, you will take a leading role in patient care with respect to eye and vision care as well as general health and well-being.

With the autonomy to establish your days of practice, hours of operation, patient schedule, and fee schedule, you will have the freedom to run your practice as you see fit.

As an independent lease holder, you will be responsible for:

  • Establishing your days of practice
  • Establishing your hours of operation
  • Controlling your patient schedule
  • Setting your fee schedule
  • Keeping 100% of exam/professional fees

You will also have the option to opt into equipment/instrumentation and administrative and/or Opt-Tech support services for a fair market value.

To be eligible for a lease agreement, you must meet the following minimum qualifications:

  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school
  • Valid state ophthalmology license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
