**Role Details**
This exciting opportunity involves working as a part-time Complex Care Specialist in our New Mexico location. You will be responsible for providing high-quality, comprehensive primary care services to patients managing complex and chronic illnesses. Using our innovative, proprietary app and technology, you will work closely with patients to address their unique needs and develop personalized care plans.

We seek candidates with 4+ years of clinical experience in complex care, chronic care management, or primary care, along with experience in digital-first healthcare. Familiarity with our current tech stack, including proprietary built clinician-patient platform, Google Suite, and Slack, is highly desired. Proficiency in English/Spanish (reading, writing, speaking, and understanding cultural nuances) for native Spanish speakers in a medical setting is also advantageous.

Key Responsibilities
  1. Provide high-quality, evidence-based primary care services to patients using our innovative, proprietary app and technology.
  2. Develop and implement personalized care plans tailored to each patient's unique needs.
  3. Cultivate strong relationships with patients, fostering trust and open communication.
