Home Healthcare Nurse Leader

This is an exciting opportunity for a compassionate and skilled Hospice Registered Nurse to join our team at Fortis Health. As a key member of our healthcare team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals. You will also have the ability to prioritize tasks and manage your time effectively in a fast-paced environment.
Key Responsibilities- Admission Assessment: Perform admission assessment within 24-48 hours of referral or patient return to residence per agency guidelines.
- Plan of Care: Develop a written plan of care for each patient.
- OASIS Data Collection: Collect accurate OASIS data at time appropriate periods.
- Patient Care: Assess patient's condition, initiate plan of care, re-evaluate and update as necessary.
- Pain Management: Able to adequately assess and reassess pain. Utilize appropriate pain management techniques. Educate patient and family regarding pain management.
- Medication Administration: Demonstrate knowledge of medications and their correct administration based on age of patient and patient's clinical position.
- Clinical Judgment: Follow seven medication rights and reduce potential for medication errors.
- Collaboration: Provide skilled nursing care. Obtain all necessary physician orders and collaborate with physician as patient's needs dictate.
- Safety and Quality: Perform all aspects of patient care in environment that optimizes patient safety and reduces likelihood of medical/healthcare errors. Support and maintain culture of safety and quality.
- Communication: Initiate case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
- Documentation: Complete patient documentation on time.
- Professional Development: Attend in-services and staff meetings as needed. Complete assigned training on time. Complete annual education requirements.
- Confidentiality: Always maintain patient confidentiality.
- Additional Duties: Perform other duties as assigned.
