**Key Responsibilities:**

* Administer medication to members according to individual plan of care, physician's orders, and emergent needs
* Perform assessments and observe and monitor members' vital signs and responses to treatments and medications
* Coordinate care with other member care services, including dietary, pharmacy, dental, rehabilitation, physical/speech/occupational therapy, audiology, and DME
* Assist members with daily functional activities, including hygiene, meals, ambulation, and social activities

**Qualifications:**

* HS Diploma or Equivalent
* Current LPN License, CPR certification, and valid malpractice insurance policy
* Min. 1 year experience in a licensed healthcare facility
* Prior experience working with the elderly and/or adults with disabilities preferred

**Physical Demands:**

* Push, pull, squat, turn, pivot, transfer, and lift at least 50 pounds
* Sit, stand, and/or walk for extended periods of time while providing physical assistance to members
