Job Summary

The Case Manager plays a vital role in promoting optimal social and emotional functioning, enabling patients to effectively utilize healthcare services and achieve their optimal level of health. By recognizing the relationship between psychosocial factors and illness, the Case Manager identifies variables that can impact a patient's recovery, collaborating with the interdisciplinary team, patients, and loved ones to mobilize community services and personal resources. This results in reduced hospital and patient costs, decreased readmissions, and improved quality of life for patients.

Key Responsibilities
  • Discharge screening, evaluation, and implementation of discharge plans, focusing on patients with complex needs.
  • Reassessment of discharge plans and monitoring for changes in condition.
  • Patient advocacy with support persons and healthcare teams as needed.
  • Interventions and support for family stress and emotional needs related to healthcare decisions.
  • Crisis intervention and adjustment to illness or bereavement.
  • Resource brokering (SNF, home care, hospice, infusion, LTAC, DME, etc.).
  • System integration and continuity of care, with interdisciplinary collaboration.
  • Adoptions.
  • Follow-up calls for Transitions of Care Discharge Policy, assessing patient care needs over the phone and applying interventions.
  • Provision of resources to patients prior to or after admission to facilitate effective discharge plans.
  • Chemical dependency and mental health assessments, with referrals to treatment.
  • Readmission assessments and trending.
  • Provision of Medicare Notification Letters.
  • Assistance with financial concerns, linking to eligible programs and hospital assistance programs.
  • Abuse assessments and referrals.
  • Assessment of Social Determinants of Health and ability to understand government programs and social service agencies.
  • Provision of information and execution of advance directives.
  • Participation in departmental, hospital, and community meetings and initiatives relative to departmental scope.
Requirements

A Master of Social Work degree (M.S.W.) from an accredited social work program, with either:

  • Education emphasis on healthcare, including a field placement in a hospital social service department or other healthcare agency.
  • Previous work experience in a hospital social service department or other healthcare setting.
  • Behavioral health experience preferred.

Mandatory licensure/certifications:

  • Valid and active Indiana Social Work licensure required.

Work Shift: 1st Shift (United States of America)

Scheduled Weekly Hours: 40
