About the Role

We are seeking an experienced and compassionate Nurse Care Manager to join our team at D-H Lebanon-MHMH. As a key member of our nursing staff, you will play a critical role in ensuring the high-quality care and well-being of our residents.

Job Summary

The Nurse Care Manager will be responsible for managing and supervising resident associates, conducting assessments, and orienting new residents to our facility. You will also assist in the investigation of incidents, identify safety hazards, and implement corrective actions. Additionally, you will monitor and maintain records reflecting residents' physical and mental condition, report significant changes to physicians, and document medication supervised and administered.

Responsibilities
  • Conduct and maintain resident's assessments using the state assessment tool for assisted living
  • Promote accuracy and participate in the process of monthly medication records
  • Plan and delegate work assignments during shift
  • Assist in conducting admission assessments and orient new residents to the facility
  • Demonstrate an appreciation of the heritage, values, and wisdom of the residents and an understanding of the aging process
  • Assist in the investigation of resident and employee incidents, identifies safety hazards, and institutes corrective action in a timely manner
  • Share in the responsibility for providing and maintaining an environment that is conducive to the safety and physical and emotional well-being of residents and personnel
  • Monitor and maintains records reflecting residents' physical and mental condition
  • Reports significant changes in a resident's condition to his or her physician
  • Documents medication supervised and administered and notes resident's response
  • Follows through on verbal medication change orders received from the physician
  • Documents resident incidents
  • Demonstrates ability to function as a team member in sharing responsibilities for the administration of resident services in cooperation with all members of the team
  • Assists in orientation of new staff
  • Provides ongoing training of staff in order to provide high quality service consistent with the philosophy of assisted living
  • Promotes and facilitates interactive problem solving with staff
  • Demonstrate basic knowledge regarding Lifeline and PHB system, initiate problem solving if needed
Requirements

To be successful in this role, you will need:

  • A graduate degree in nursing from an accredited school of nursing
  • A current New Hampshire LPN/RN license
  • Excellent interpersonal relation skills and ability to relate to a variety of people and personalities
  • Initiative, judgment, and leadership skills
  • Ability to direct and manage personnel
  • Strong organizational and time-management skills
  • Willingness to learn
  • Current BCLS certification within 30 days of hire
