Job Summary:

We are seeking a skilled Patient Care Coordinator to join our team in the Inpatient Hospital Setting. As a key member of our interdisciplinary team, you will be responsible for facilitating patient care, ensuring timely and effective transitions between levels of care, and promoting cost-effective care delivery.

Responsibilities:

  • Assess patients' healthcare needs and goals, identifying opportunities for intervention to enhance clinical outcomes and patient satisfaction.
  • Develop and implement individualized transition plans, incorporating risk assessment and socioeconomic factors to ensure safe and effective discharge.
  • Collaborate with attending physicians, nursing staff, and ancillary personnel to eliminate barriers to efficient care delivery.
  • Serve as a liaison between the interdisciplinary care team, community providers, payers, and patients/families to ensure seamless transitions between internal and external levels of care.

Requirements:

  • Bachelor of Science degree in nursing or equivalent experience required.
  • Minimum 1-3 years of hospital or related experience preferred.
  • Able to navigate and connect successfully with outside provider networks (Health Plans, IPA's, and FQHC's).
  • Current California Nursing license and Basic Life Support certification required.
  • Certification in Case Management preferred.

Estimated Salary Range: $65,000 - $85,000 per year, depending on experience and qualifications.
